Open Water Diver Registration

Before you go underwater, we need to ensure you are physically healthy to dive. Please review the following medical based questions to ensure your safety:

Do you have any YES'S? If so, before you can dive, in the interest of YOUR safety, your doctor will need to determine whether you may dive. Having a yes, does not mean you can't dive, it just means we need to double check to make sure. Please download this form and take to a physician for his/her authorization. Once you have this document signed by your physician, you may register for a scuba course below.
